Model Form for Lip Filler and Botox Treatments Classes

Thank you for your interest in participating as a model in our specialized training classes for lip filler and Botox treatments. Please read the following instructions and requirements carefully to ensure you are eligible and properly prepared for the application process.

Eligibility and Application Process:

• Initial Application: To be considered for participation, you must first complete and submit this application form. Ensure all information is accurate and that you include the required photographs taken in good lighting.
• Approval Notification: After reviewing your application, we will notify you if you have been selected. Only approved candidates will proceed to the next steps.
• Payment Instructions: Upon approval, you will be required to make a payment via Zelle to secure your spot as a model in the class. Please include your full name in the payment description for identification. Note: Do not send any payment until you receive official confirmation of your approval.
• Discounted Procedure Rate: As a model, you are entitled to a 50% discount off the standard cost of the procedure. This discounted rate is only available to participants in the training class.

Specific Treatment Criteria:

• Botox Models: To participate in the Botox course, you must not have received any Botox treatments within the last 6 months.
• Lip Filler Models: To be eligible for the lip filler course, you must not have had lip fillers before.
